Past incidents
- Impact
- Bursts of 500 errors on signed-in pages and actions, PDF imports included. The public pages were not affected.
- Cause
- The server checks every signed-in request against signing keys it caches for five minutes. When that cache expired, each request in flight fetched the keys again at the same moment, the burst tripped the sign-in service’s rate limit, and the requests waiting on the failed fetch answered with an error.
- Fix
- Key refreshes now run one at a time, a failed refresh keeps the last good keys for 15 seconds instead of failing every request behind it, and the key endpoint got the same rate limit as session checks. The fix shipped to production the same evening.
